Requirements
of trainees
All training must be co-ordinated & approved by the local
postgraduate Dean; Regional Advisor and Program Director as well
as the Endocrine CSAC of the RCPCH.
In order to gain the necessary depth of experience each trainee
should be actively involved in the management care of at least
1,000 different patients during the whole period of his/her specialty
training. This should include the care of outpatient and inpatients
and patients with endocrine emergencies where possible.
The trainee will be required to keep his/her personal log book
or equivalent up-to-date according to national guidelines and
European Union directives. The log book must be endorsed by his/her
tutor or authorised deputy.
The trainee should attend and provide evidence of attendance
at local, regional and national Endocrine and Diabetes Meetings.
Attendance at International Meetings is not essential for the
Secondary course but is considered essential for the Tertiary
course. At least 2 presentations at these meetings should be made
by a Secondarycourse member and 3 for an Tertiary course student.
Participation in one Audit project for Secondary course and two
for Tertiary course students. Tertiary course students should
conduct one systematic style review of a topic and in addition
prepare a detailed evidence based appraisal of a diagnostic test
or a therapeutic intervention to be submitted to the CSAC for
assessment and published electronically.
The Trainee is encouraged to provide as evidence of training
a series of Signs of Personal Development. These should include:-
- Broad experience of endocrinology within a single centre
and between centres
including work in other countries.
- Attainment of Higher Degrees (MD or PhD).
- Creation of Trainee Portfolio which would include, log-book,
abstracts of work presented, reviews, self directed learning
activities (SDLA) and evidence based appraisals. SDLA includes
reading activities, tutor discussions, self teaching projects,
teaching and presentation and participation in computer/distance
based learning schemes.
Criteria for assessment of trainees
Introduction
Trainees fall into 3 broad categories.
- General Paediatricians or those training for other subspecialties
requiring basic endocrine knowledge.
No CSAC assessment is required for this group although they
should acquire their training in a unit providing high quality
care and experience.
- Paediatricians training to secondary (with an
interest) level in endocrinology &/or diabetes.
There is, as yet, no formal recognition of this level of training
but CSAC should be involved in providing guidance and approval
of training. The details of the training required will depend
on the consultant post applied for it is possible that
the post may just require paediatric diabetes care rather than
diabetes & endocrinology.
- Paediatricians training to tertiary (referral
centre) level in endocrinology &/or diabetes.
CSAC is directly involved with the assessment of training providing
signature at the satisfactory completion of subspecialty training.
The detail of training may vary in emphasis depending on the trainees
primary interest (diabetes / endocrinology) but will cover the
majority of both areas.
Detail of process of assessment
Trainees in category A. do not require CSAC involvement
but should complete 6 months minimum as an SpR in a paediatric
endocrine / diabetes post.
Trainees in category B. do require informal CSAC advice
and possibly a letter of documentation at completion of CCST to
state they have trained to "secondary level (with an interest)"
in paediatric endocrinology &/or diabetes. This is supplied
after review of their C.V. and consultation with their endocrine
trainer.
In order to achieve such training they would require a minimum
of two years training at SpR level in paediatric endocrinology
&/or diabetes. Training in the rarer aspects of paediatric
endocrinology are not required but full training in paediatric
diabetes including "adult experience" is required to
fulfil the requirements of secondary paediatric diabetes care.
Trainees in category C. require formal CSAC involvement
throughout their training.
- Advice at entry.
- Allocation to recognised training posts.
- Advice during training with regards to areas of deficiency
in training requiring correction involvement of a CSAC member
in their penultimate year RITA.
- Assessment in final year of completion of training.
Tertiary level trainees are likely to be mainly in paediatric
endocrinology rather than diabetes as diabetes is mainly a secondary
level subspecialty. Secondary diabetes training
is, however, required for all tertiary trainees. Tertiary level
trainees are expected to complete the syllabus which will require
a minimum of two years SpR clinical training and
at least one year out of the formal SpR system to undertake research
/ fellowship and in most cases to gain a higher degree.
Assessment by the CSAC will be by
- The trainees detailed Curriculum Vitae including details of
clinical experience (see syllabus subheadings), research and
publications.
- Report from the trainees supervisor and a letter from
the Programme Director for confirmation of the accuracy of the
trainees C.V..
- In some cases by interview with the trainee.
The completion of subspecialty training form will be signed of
upon satisfactory completion of the above.
